We are searching for a compassionate and experienced Social Services Director to lead our social services programs.

Responsibilities:


* Lead the planning and implementation of resident-focused social services.


* Offer emotional support and counseling to residents and their families.


* Manage and coordinate the discharge planning process with the care team and external agencies.


* Schedule and arrange all necessary on-site and off-site resident services.


* Oversee practical needs, including personal shopping and resident belongings management.


* Maintain effective, empathetic communication with all stakeholders.

Qualifications:


* Bachelor's degree in Social Work or a related field (MSW a plus).


* 2+ years of supervised social work experience in a healthcare setting.


* Ability to handle complex, confidential situations.


* Strong organizational and communication skills.


* Proactive and persistent approach to problem-solving.
