Residential Nurse Case Manager - Pittsburgh (LPN)

Description:

The Residential Nurse Case Manager plays a vital role in ensuring the health and well-being of individuals with intellectual disabilities in residential group homes and Adult Services Programs.

This position involves developing and managing individualized healthcare plans, coordinating medical care, and collaborating with interdisciplinary teams to support holistic health outcomes.

What You’ll Have:


* Licensed Practical Nurse (LPN) certification in Pennsylvania


* At least 1 year of clinical experience in acute or long-term care


* Preferred experience working with individuals with intellectual disabilities


* CPR and First Aid certification (preferred)


* Ability to manage confidential information with discretion


* Strong organizational and time management skills

A Typical Day May Include:


* Visiting group homes to assess health and safety needs


* Coordinating medical appointments and transcribing physician orders


* Monitoring medication administration and liaising with pharmacies


* Conducting quarterly nursing assessments and staff trainings


* Participating in interdisciplinary meetings and care planning


* Maintaining comprehensive medical records and ensuring timely communication


* Responding to after-hours calls related to electronic medication records (eMAR)


* Attending in-service programs and completing continuing education requirements
