Liberty Resources is currently seeking a Substance Use Disorder Counselor for our Integrated Healthcare Clinic.

Position Summary:

The Substance Use Disorder Counselor is primarily responsible for the provision of therapy from a holistic, dual-recovery lens to individuals, couples, families and groups referred to the Integrated Health Care Clinic.

Job Responsibilities:


* Provides person centered recovery focused outpatient mental health care to individuals, couples, families and groups.


* Conducts intake assessments, including gathering data from spouses, significant others, parents, children and other relevant sources.


* Utilizes diagnostic and assessment information to develop clinical case conceptualization and treatment plans in collaboration with client system and clinical supervisor.


* Provides crisis intervention services when necessary for caseload (on call required)


* Create person centered, recovery focused treatment plans with measurable goals and objectives.


* Advocates on behalf of the client (adult, children and their families) to ensure adequacy of services (i.e.

educational, legal and community) by providing linkages and coordination with county providers and agencies.


* Carries out Agency policies and procedures.


* Completes all necessary written documentation, reports, statistics, case files, treatment plans and summaries in a timely manner and in accordance with program (OMH/AAPC) and agency standards. 


* Attends and participates in individual supervisory conferences, staff meetings, training sessions and professional development opportunities as required.  Maintains required program and professional licenses/certifications.


* Handles emergency situations which may require flexibility of time and area of assignment.


* Provides clinical services one evening per week until 7:00 p.m.

unless authorized by the VP/Designee.


* Performs other duties, responsibilities and related tasks as assigned.
