Job Description:


* Treatment Plan Development: Develop effective treatment plans and obtain necessary approvals from referring physicians.


* Patient Care: Treat patients according to physician treatment plans.


* Supervision: Supervise Physical Therapy Assistants in direct patient care and related activities, following state practice acts.


* Collaboration: Communicate with supervisors and other health team members regarding patient progress, problems, and care plans.


* Meetings Participation: Attend Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.


* Documentation: Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with PT Board requirements and policies.


* Family Instruction: Instruct patients' families or nursing staff in maintenance programs and caregiver training in preparation for discharge.


* Discharge Planning: Participate actively in discharge planning processes.


* Recommendation of Equipment: Recommend and facilitate the ordering of necessary durable medical equipment to enhance patient mobility and independence.


* Equipment Maintenance: Report issues with department equipment to ensure it is maintained in good working order.

Required License or Certification:


* Valid Physical Therapist License
