Are you a dedicated and compassionate LVN looking for flexibility and a team that values your skill and heart for care? Lincoln Meadows Care Center is seeking an On-Call Licensed Vocational Nurse to join our professional and caring nursing team!

As an LVN at Lincoln Meadows, you'll play a vital role in ensuring our residents receive the highest quality of care in a warm and supportive environment.

What You'll Do:


* Provide direct nursing care to residents in accordance with physician orders and care plans.


* Supervise and support Certified Nursing Assistants (CNAs) during your shift.


* Administer medications and treatments safely and accurately.


* Monitor resident conditions and report changes to the supervising nurse or physician.


* Maintain accurate and timely documentation and charting.


* Promote resident dignity, comfort, and well-being at all times.


* Ensure a clean, safe, and organized work environment that complies with infection control standards.


* Collaborate with the care team to support resident-centered outcomes and continuity of care.

What We're Looking For:
✅ Current and in good standing LVN license in the State of California
✅ Excellent communication and leadership skills
✅ Strong attention to detail and ability to multitask
✅ Compassionate, dependable, and team-oriented
✅ Experience in long-term care or skilled nursing preferred
