Nurse Manager

Essential Duties


* Care, Coordination, & Oversight
+ Develop, update, and monitor individualized resident care plans in collaboration with the interdisciplinary team.
+ Ensure that care plans reflect the physical, emotional, and psychosocial needs of each resident.
+ Conduct routine assessments and communicate changes in condition to the physician and appropriate staff.


* Resident Care & Advocacy
+ Monitor resident health status and provide direct nursing care as needed.
+ Advocate for residents' rights and quality of life, ensuring dignity, respect, and safety.
+ Act as the primary point of contact for families regarding resident care updates.


* Team Leadership & Supervision
+ Supervise and support CNAs and other nursing staff in the delivery of daily care.
+ Provide training, guidance, and feedback to staff to ensure adherence to care standards and policies.
+ Assist in performance evaluations and corrective actions when necessary.


* Documentation & Compliance
+ Accurately document evaluations, treatments, and care interventions in the medical record.
+ Maintain compliance with state, federal, and facility regulations, including infection control and safety protocols.
+ Prepare for and participate in state surveys, audits, and quality assurance activities.
+ Complete MDS information on residents as required.


* Communication & Collaboration
+ Coordinate with physicians, therapists, and other healthcare professionals to ensure continuity of care.
+ Facilitate care conferences with residents and families.
+ Report significant changes, incidents, or concerns promptly to the Director of Nursing.

Qualifications


* Education and/or Experience
+ Current, unencumbered Registered Nurse (RN) license in the state of practice.
+ Minimum of 2 years of nursing experience in a skilled nursing, long-term care, or similar healthcare setting preferred.
+ Strong knowledge of state and federal long-term care regulations.


* Language Skills
+ Excellent organizational, communication, and leadership skills.


* Reasoning Ability
+ Ability to manage multiple priorities and adapt to changing resident needs.


* Certificates, Licenses, and Registrations
+ Current, unencumbered Registered Nurse (RN) license in the state of practice.

Physical Demands


* Ability to lift, push, or pull up to 50 lbs.


* Extended periods of standing, walking, and moving throughout the facility.


* Must be able to respond quickly in emergency situations.
