Pay: $17.69/hour

Work Schedule: Monday-Friday; Part-Time

Part-Time position is eligible for direct primary care coverage.  In addition, these positions are eligible for paid time off in the form of vacation, sick, holiday, floating holiday, jury duty, and bereavement leave.

All employees may also make pre-tax or post-tax (Roth) contributions to our retirement plan – must be 18 years of age to participate.

JOB SUMMARY:

The Personal Care Worker will increase the quality of life through promotion of independence within the home by assisting with light homemaking, non-medical personal care, home management, and emotional support.

The Personal Care Worker will perform direct care services to disabled and or elderly individuals in the community or in their home.

ESSENTIAL FUNCTIONS:

Duties of a personal care worker may include:


* Observation and maintenance of the home environment and ensures the safety and security of the client.


* Assistance with household chores including cooking and meal preparation, cleaning, and laundry.


* Assistance in completing activities such as shopping, and appointments outside the home.


* Companionship including, but not limited to, social interaction, conversation, emotional reassurance, encouragement of reading, writing and activities that stimulate the mind.


* Assistance with activities of daily living, personal care and any other assignments as included in the service plan.


* Completion of appropriate service notes regarding service provision for each visit.

Documentation shall contain services provided, date and time in and out, and a confirmation that care was provided.

Client Care:


* The Personal Care Worker will plan with the clients and their families on how best to carry out the required tasks and functions, organize the tasks within the care plan or Individual Support Plan (ISP) and perform the required tasks.

The incumbent will keep appropriate records, assist with activities, meals, and snacks, and communicate with the Supervisor regarding medical concerns and any changes that must be made to the ISP or Care plan.

The incumbent will recognize the importance of a clean, healthy environment in performing household tasks; take measures to keep the home in a safe condition and respect the confidentiality of the client.

Service Delivery:


* The Personal Care Worker will provide services that are listed in the ISP or Care Plan.
