Crisis Counselor (Mobile Crisis Response)

$25.38 - 29.18 /Hr.
Schedule: Wednesday - Saturday; 7:00 am - 5:30 pm
Location: Rogers Park

Trilogy has received funding from the State of Illinois to develop and implement mobile crisis response program in response to the recognition that mental health crises are not currently addressed in the most effective, safe, and trauma-informed way.  Trilogy will provide mobile crisis outreach services to community members in our designated CCBHC and MCRT zip codes. 

The Crisis Counselor is a Qualified Mental Health Professional (QMHP) or Mental Health Professional (MHP) responsible for providing immediate, on-site clinical assessments for individuals experiencing a mental health crisis.

As part of a mobile crisis response team, the Crisis Counselor evaluates the client's mental status, safety risks, and psychosocial needs to determine the most appropriate level of care, including hospitalization, outpatient services, or community-based support.

This role involves crisis intervention, risk assessment, de-escalation, and coordination with other service providers to ensure timely and effective care.  

 

Responsibilities: 


* Conduct clinical assessments in the field in pairs with a Peer Engagement Specialist to evaluate mental health status, safety risks, and psychosocial needs.


* Determine appropriate level of care based on assessment findings, ranging from safety planning and outpatient referral to involuntary or voluntary hospitalization.


* Provide immediate crisis intervention and de-escalation using trauma-informed and culturally responsive approaches.


* Collaborate with team members, including the Peer Engagement Specialist and clinical leadership, especially when consultation or clinical support is needed.


* Develop and implement individualized crisis response and safety plans that prioritize client stability and least-restrictive care options.


* Collaborate with external community partners, such as law enforcement, EMS, and other first responders, to ensure a safe, timely, and effective response.


* Communicate with hospitals, outpatient providers, and community agencies to facilitate timely referrals and transitions of care. 


* Complete all documentation in a timely and thorough manner.


* Participate in team meetings, debriefings, and trainings.


* Maintain knowledge of community resources, clinical best practices, and applicable laws and regulations related to Mobile Crisis Response Teams.


* Perform other related duties and/or projects as assigned. 

Qualifications: 


* Must have one of the following requirements: 
+ Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field
+ Bachelor's degree in any other field with two years of supervised clinical experience in a mental health setting
