Community Health Worker/Promotore II - Spanish

Bilingual in Spanish is required

The County of Riverside is seeking a Bilingual Community Health Worker/Promotore to join our team.

We are currently recruiting for a Community Health Worker/Promotore II to support our Family resources locations in Desert Hot Springs and Mecca.

The incumbent will be responsible for driving throughout Riverside County to attend various event fairs, representing and offering programs across the region.

They will play a key role in connecting with community members to help them access essential health services and resources, including assistance with health program applications.

Responsibilities will include, but not limited to, conducting outreach and interviews to assess community needs while providing tailored support to individuals and families, particularly in underserved populations, and provide resources between the public and health/social services to promote health and well-being programs offered with the County.

Competitive candidates will possess a Community Health Worker certification, strong communication skills, and a solid understanding of programs and community outreach within healthcare and human services programs, and high volume-case management.

jfc: 21109100• Build rapport with clients and gather, share, and assess accurate information to identify efficient and effective opportunities for intervention utilizing strategies that reflect the client's values, attitudes, and beliefs.

• Provide health education, instruction, and information in a manner that clients can easily understand and address barriers to physical and mental health services.

• Assist clients in navigating the healthcare system and public health programs by phone and in person to help clients successfully participate in their medical and/or behavioral care.

• Engage with community members to raise awareness about violence prevention strategies.

• Build trusting relationships with individuals and families affected by asthma, offering guidance, emotional support, and practical assistance.

• Connect program participants to community resources necessary to achieve healthy outcomes; address healthcare barriers and identify and address health-related social needs; make closed-loop referrals to local human service systems to support clients address their needs includin...
