MDS Assistant Nurse - LVN

Tice Valley Post Acute is seeking a Licensed Vocational Nurse (LVN) to serve as an MDS Assistant.

This is a full-time position with a competitive salary range of $42-$45 per hour, depending on experience.

Responsibilities:


* Oversee weekly PDPM meetings for skilled patients at the facility.


* Track and monitor the effective utilization of PDPM tools, including Physician Diagnosis Verification, Admission/Discharge CSR (GG), Weekly CSR, PDPM Weekly Tracking Tool, and Point Right.


* Monitor QM and QASP performance monthly and quarterly, providing data analysis and clinical recommendations for optimal outcomes.


* Train new MDS Coordinators on PDPM and MDS processes.


* Assess and determine the health status and level of care for all new admissions.


* Ensure the accurate and timely completion of all MDS Assessments, including PPS Medicare, quarterly, annual, and significant change assessments.


* Communicate the level of care for new residents to all relevant disciplines.


* Coordinate interdisciplinary participation in completing MDS for new admissions in accordance with regulatory timelines.


* Ensure documentation completeness and thoroughness as mandated by federal, state, and medical standards.

Certificates, Licenses, Registrations:


* Proficiency in Microsoft Suite products is required.


* Knowledge and experience with PCC is preferred.


* Must maintain all required continuing education and remain in good standing with the State Board of Nursing.
