Permanent, full time 37.5 hours, rota working including weekends

Pay £13.21 per hour (£25,838 per annum) and great benefits including Health Cash Plan

Want to be part of a team that really cares and empowers you to help change a customer’s life for the better?  As our Support Coordinator, you’ll be at the heart of supporting our customers who have complex needs to develop their skills to live more independently in the community.  You’ll make a big difference each day as you help our customers achieve their hopes and aspirations.  

Typical day as a Support Coordinator


* Creating support plans with your customers and coordinating our small team of Support Workers working to the plans. 


* Customer group - Single homeless men with low to medium support needs


* Supporting customers with a variety of needs such as, budgeting, maintaining their tenancy, substance misuse, offending and moving onto independent accommodation.


* Carrying out risk assessments, support planning, goal setting and regular reviews.


* Cuppas, chats and catch ups.  There’s no rushing from one customer to the next here!  Plenty of time to collaborate with colleagues too.

You bring


* Passion to support our customers to live their best life, working collaboratively with an eye for detail. 


* Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.


* Experience of coordinating and assessing customer referrals.


* The ability to work on your own initiative, remain calm under pressure and have a resilient approach

 

Job details


* Where we’re flexible with you, we do need to meet the needs of our customers.

That may mean adapting your start or finishing times on occasion.


* Work pattern will be on a rota basis including some weekends


* Although you’ll lone work at times, we do get together regularly [in the daytime] for training and team meetings.


* Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.


* You’ll need an Enhanced DBS check done and we pay for that.
