Diablo Valley Post Acute a premier provider of rehabilitation and long-term care in Contra Costa County, is seeking a compassionate Certified Occupational Therapist Assistant (COTA) to join our dynamic and passionate team.

Key Responsibilities:


* Treat patients as directed by the Occupational Therapist and per the physician's treatment plan.


* Document evaluations, daily treatment notes, weekly progress notes, and recertification documentation in compliance with the OT Board State Practice Act and payer requirements.


* Train restorative aides to ensure continuity of care.


* Communicate patient progress, problems, and plans with the interdisciplinary healthcare team.


* Educate patients, families, and nursing staff on maintenance programs and caregiving techniques for discharge planning.


* Assist with the cleaning and maintenance of the treatment area and departmental equipment.


* Relate positively and effectively with patients, families, and professional colleagues.

Qualifications:


* Valid Certified Occupational Therapist Assistant (COTA) license in the state of California.
