Compass Health is currently seeking a Clinician II - Mental Health Professional to join our PACT North team based in Mt.

Vernon.

This is an opportunity to engage deeply with clients through a community-centered, multi-disciplinary care model that promotes recovery, resilience, and independent living.
About the Program
PACT (Program for Assertive Community Treatment) is an evidence-based, intensive outpatient service designed for individuals with severe and persistent behavioral health challenges.

Our team-based model includes mental health professionals, a psychiatric prescriber, nurses, vocational and substance use specialists, peer support, and care coordinators.

Services are provided primarily in the community, including clients' homes, and support is available 24/7 to meet individual needs.
What You'll Do


* Provide recovery-focused services including case management, treatment planning, resource navigation, and advocacy.


* Conduct risk assessments and develop individualized care plans.


* Deliver services in the community, meeting clients where they are.


* Collaborate closely with the multi-disciplinary PACT team to ensure wraparound, coordinated care.


* Participate in 24/7 on-call rotation and crisis response.


* Maintain documentation and records in compliance with agency and regulatory standards.

What You Bring


* MA/MS/MSW in a behavioral science field.


* Ability to obtain WA State Agency Affiliated Counselor registration (if not already credentialed).


* Valid WA State driver's license, vehicle, and appropriate insurance (as applicable).


* Commitment to working evenings, weekends, and holidays as needed.


* Strong clinical judgment and teamwork skills.

* Experience working with individuals experiencing severe behavioral health conditions.


* Demonstrated ability to navigate complex systems and provide strengths-based care.


* Crisis management and de-escalation skills.


* Excellent written and verbal communication.


* Familiarity with electronic medical record (EMR) systems.
