Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN)
Job Summary
The Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) is responsible for directing the daily functions of nursing assistants and ensuring the overall care of residents is provided in accordance with facility policies and government regulations.
This role involves a mix of administrative duties, direct nursing care, and staff supervision to maintain a safe and high-quality environment for all residents.
Essential Duties and Responsibilities
Administrative & Supervisory
* Direct Daily Operations: Supervise nursing assistants and ensure they follow facility policies, procedures, and job descriptions.
* Reporting & Documentation: Make both written and oral reports on your shift's activities and complete necessary administrative tasks such as accident/incident reports, resident charge slips, and medical forms.
* Staff Management: Participate in performance evaluations, make recommendations on staffing needs, and provide leadership to your team.
* Inter-departmental Coordination: Cooperate with other departments to ensure a resident's total care plan is maintained and to adequately plan for resident needs.
* Policy & Procedure: Help interpret and enforce department policies for staff, residents, and visitors.
Participate in the development and maintenance of the facility's quality assurance program.
Charting & Documentation
* Record Keeping: Complete and file all required forms upon a resident's admission, transfer, or discharge.
* Physician Orders: Receive and transcribe telephone orders from physicians and ensure they are accurately documented in resident charts and care plans.
* Nurses' Notes: Write informative and descriptive notes that reflect the care provided and the resident's response to it.
* Confidentiality: Maintain the confidentiality of all resident information and report any suspected violations.
Nursing Care & Drug Administration
* Medication Administration: Prepare and administer medications as ordered by a physician, ensuring proper resident identification and adherence to stop order policies.
* Direct Care: Administer professional services such as catheterization, tube feedings, wound care, and specimen collection.
* Resident Monitoring: Make periodic checks to ensure treatments are being administered correctly and to evaluate the resident's physical and emotional status.
* Communication: Notify the attending physician and family members of any changes in a resident's condition or if they are involved in an accident.
* Restorative Care: Carry out restorative and rehabilitative programs as instructed.
Safety & Sanitation
* Infection Control: Ensure staff follow established handwashing and infection control procedures, especially when isolation precautions are necessary.
