* Part-Time: Thursday - Friday, 8:00 AM - 8:00 PM
Location: Sullivan Park Care Center
Position Summary
Sullivan Park Care Center is seeking dedicated and detail-oriented Licensed Practical Nurses (LPNs) to join our Medicare team.
Key Responsibilities
* Complete and follow up on MD orders in collaboration with the interdisciplinary team.
* Document daily skilled nursing notes in accordance with Medicare guidelines.
* Perform and record weights and nutrition at risk.
* Conduct comprehensive resident evaluations, ensuring timely assessments and updates.
* Lead and participate in infection prevention protocols, including monitoring, reporting, and staff education.
* Maintain compliance with all federal, state, and facility policies, procedures, and documentation standards.
Qualifications
* Current LPN license in the state of Washington.
* Prior experience in skilled nursing, long-term care, or Medicare documentation strongly preferred.
* Strong clinical assessment, organizational, and time-management skills.
* Knowledge of MDS processes and Medicare regulations is a plus.
* Ability to work independently and as part of a team.
* Excellent communication and documentation skills.
