MDS Nurse

Peninsula Post Acute is looking for a dedicated and detail-oriented MDS Nurse (LVN) to join their team.

About the role:

This crucial role focuses on managing the Minimum Data Set (MDS) process, which is essential for resident care planning and facility compliance with federal and state regulations.

You'll play a vital role in ensuring residents receive the highest quality of care and that accurate documentation supports the facility's operations.

Key responsibilities:


* Collecting and completing MDS documents within required timeframes.


* Reviewing and comparing entered data against original documents to ensure accuracy and making necessary corrections.


* Maintaining organized and up-to-date resident charts by filing MDS documents appropriately.


* Protecting sensitive patient care information and maintaining strict confidentiality at all times.


* Collaborating with the interdisciplinary team to develop, implement, and evaluate personalized care plans.


* Ensuring compliance with federal and state regulations, including PDPM guidelines.

What you bring:


* Current and active Licensed Vocational Nurse (LVN) license.


* Strong understanding of MDS regulations and processes.


* Excellent assessment, organizational, and critical thinking skills.


* Proficiency in maintaining accurate medical records and documentation.
