? What You'll Do

As our Discharge Planner, you'll be the bridge between care and independence, helping residents transition smoothly from our facility to their next destination—whether it's home, assisted living, or another care setting.

Your role will include:


* Coordinating discharge plans with residents, families, and interdisciplinary teams


* Ensuring all medical, social, and logistical needs are met for a safe transition


* Communicating with external providers and community resources


* Educating residents and families on post-discharge care and services


* Keeping documentation accurate, timely, and compliant

* Experience in discharge planning, case management, or social services


* LVN license preferred (but not required)


* Strong communication and organizational skills
