Key Responsibilities


* Complete and coordinate the Minimum Data Set (MDS) process in compliance with state, federal, and facility guidelines


* Accurately assess and document resident needs and conditions


* Collaborate with the interdisciplinary team to ensure care plans reflect resident goals and clinical requirements


* Monitor quality measures and ensure accurate reimbursement through proper coding and documentation


* Assist in training staff on MDS-related protocols and updates


* Support the Director of Nursing and leadership team in driving quality outcomes and regulatory compliance

Qualifications


* Active California LVN or RN license required


* Minimum of 1-2 years of MDS experience in a skilled nursing facility preferred


* Strong knowledge of PDPM and care planning process


* Excellent clinical judgment, organizational skills, and attention to detail


* Strong communication and teamwork skills


* Passion for improving patient outcomes and ensuring accurate documentation
