Pay: $20/hour

Work Schedule: Monday - Friday, 8am - 4pm.

Some weekend/evening work may be required

JOB SUMMARY:

The Case Manager II, Programs will assist in the design and delivery of program activities for person with mental, physical, and/or intellectual/developmental disabilities.

This position will be responsible for continuous support to the Employment and Community based programs.

ESSENTIAL FUNCTIONS:

The Case Manager II, Programs will perform the following duties personally or through delegation:


* Supervises a caseload of consumers consisting of all disability areas and encourages the utilization of multiple community agencies serving persons with mental, physical, and/or intellectual/developmental disabilities. 


* Develops and maintains contact with other agency staff and local service providers to develop specialized services for consumers and their families.


* Assigns, guides, coaches, and trains subordinates in alignment with Community Employment and Day Programs, and Goodwill policy and procedures.

Provides information concerning case management practices, administrative rules and regulations, community resources, vendors, and programs.


* Serves as a resource to address sensitive consumer needs and/or accommodations.


* Develops Individual Services and Supports Plans for each consumer within the programs, implements and monitors plan progress.

Adjusts and modifies development plan as needed to meet changing consumer needs.


* Participates on the inter-disciplinary team to review each consumer’s case and participates in all inter-disciplinary team meetings for each consumer.


* Prepares or dictates case progress notes, social summaries, and other reports concerning the consumer’s case.


* Attends staff meetings concerning new or revised policies and procedures.


* Collaborates with and receives direct supervision from Manager in making decisions related to consumer services.
