Key Responsibilities:


* Evaluate and treat residents in accordance with physician orders and individualized therapy plans


* Develop comprehensive treatment goals and monitor resident progress


* Educate residents, families, and caregivers to support functional gains


* Collaborate with an interdisciplinary team to ensure the best clinical outcomes


* Maintain accurate and timely documentation in accordance with regulatory standards

Qualifications:


* Licensed Physical Therapist (PT) in the state of [Insert State]


* Degree from an accredited Physical Therapy program


* CPR certification (or ability to obtain)


* Previous experience in a skilled nursing or long-term care setting preferred
