Clinical Pharmacist – Oncology

Work Schedule:

100% FTE, day shift, between the hours of 7:00AM - 6:00PM, covering approximately 1 in 4 Saturdays.

You will work at Eastpark Medical Center and University Hospital in Madison, WI.

Hours may vary based on the operational needs of the department.

We are seeking a Clinical Pharmacist in Oncology to:


* Provide exemplary patient care in conjunction with interdisciplinary team members across oncology infusion areas and ambulatory care settings.


* Perform clinical verification of chemotherapy orders, including oral chemotherapy


* Provide and execute management strategies for hypersensivity reactions and other supportive care requirements for oncology treatments.


* Practice through delegation protocols to manage chemotherapy induced nausea and vomiting, electrolyte imbalance, potential chemotherapy toxicities, and achieve therapeutic drug levels.


* Ensure appropriate utilization of medications to optimize patient outcomes.

PGY2 in oncology pharmacy or previous work experience in oncology highly desired.

Qualifications



* Bachelor's Degree in Pharmacy or a PharmD degree Required


* Completion of an ASHP Accredited Residency training program Preferred

Work Experience



* 1 year of clinical patient care and orders management experience Required


* 3 years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited residency training program Preferred

Licenses & Certifications



* Licensed to practice pharmacy in the State of Wisconsin within 90 Days Required


* Basic Life Support (BLS) within 90 Days Required


* Certification through the appropriate certifying program (Board of Pharmacy Specialties, National Board of Nutrition Support Certification, National Certification Board for Anticoagulation P...
