The Health Home Care Manager (HHCM) Team Lead functions as a part of the leadership team to assist with the supervision and oversight of Care Managers.

The Team Lead is responsible for coaching, collaborating with care managers and providers, and when possible, educating the member and other care team members of the health benefits of closing gaps in care.

Health Home Care Manager (HHCM) Team Lead Essential Functions


* Assists Supervisor with coaching and training of Care Management Staff


* Shadows one appointment per month with each Care Manager on their team to provide coaching and ensure quality services are rendered


* Serves as a mentor to onboarding staff to provide additional support to new staff members


* Attends Health Home Hub meetings and provides feedback to leadership team


* Facilitates monthly team meetings to offer training and coaching on policies and program standards


* Identifies training needs for team and implements training and coaching for staff


* Maintains a small case load of active clients


* Assists with enrollment activities as needed

Qualifications


* Bachelor’s degree in health or human services field with at least two (2) experience is preferred; Registered Nurse with 2 years of relevant experience or Master’s degree with 1 year of relevant experience


* Must have a valid New York State driver’s license and access to reliable transportation.

Pay: $55,000
