Social Services Director - Alhambra Post Acute
Location: Martinez, CA
Pay: $20 to $28 per hour, DOE
Schedule: Full-Time

Alhambra Post Acute is looking for a Social Services Director to join our leadership team.

Responsibilities


* Oversee the social services department and supervise social services staff


* Support residents and families with psychosocial needs, care planning, and discharge planning


* Develop and maintain relationships with community resources and agencies


* Participate in resident care conferences and interdisciplinary team meetings


* Ensure compliance with state and federal regulations related to social services


* Provide education and support to residents, families, and staff regarding care transitions and available resources

Qualifications


* Degree in social work, psychology, or related human services field preferred


* One year of supervised social work experience in a healthcare setting required


* Knowledge of discharge planning processes and community resources


* Strong communication, organizational, and leadership skills


* Compassionate and resident-focused approach
