Admissions Coordintator -temporary position

Admissions Coordinator Full time, Temp position through January-possible permanent position following.

White Blossom Care Center is a premier skilled nursing facility in Salinas, is seeking a compassionate and motivated professional for a full-time, dual-role position as an Admissions Coordinator.

This role is ideal for a dedicated team player who thrives in a fast-paced environment and is passionate about providing a seamless and supportive experience for our residents and their families.

Responsibilities

Admissions and Family Support:


* Guide residents and their families through the admissions process with empathy, setting expectations and outlining care options.


* Act as a liaison between residents, families, and our interdisciplinary team to ensure a smooth transition into our facility.


* Provide emotional support to residents and families as they navigate transitions, address health-related fears, and cope with loss.


* Evaluate and assess residents' social, family, and psychosocial needs to develop a holistic care plan.


* Maintain accurate and timely documentation of admissions, transfers, and discharges.

Qualifications


* Preferred but not required: Previous experience in a skilled nursing facility.


* Familiarity with PCC (PointClickCare) or other electronic health record (EHR) software is a plus.


* Demonstrated ability to work effectively in a high-energy, fast-paced environment.


* Exceptional communication and interpersonal skills, with a positive and empathetic attitude.


* Strong teamwork skills and a commitment to collaborative patient care.
