Responsibilities


* Provide skilled physical therapy services in accordance with resident care plans


* Develop and implement individualized treatment programs


* Work closely with an interdisciplinary team to ensure optimal resident outcomes


* Document care accurately and completely

Qualifications


* Current California Physical Therapist license


* Strong clinical skills and a passion for improving lives


* Excellent communication, teamwork, and empathy


* Previous SNF, rehab, or post-acute experience preferred (new grads welcome)
