Pay: $19/hour

Work Schedule:  Monday-Friday 7:30am-4:00pm

JOB SUMMARY:

The Supervisor II, Lead Case Management, will be responsible for providing directions and guidance to Case Managers.

This individual will provide support to ensure the needs of the participants at the affiliated Goodwill of Colorado Day Program sites are being met to maximize their fullest potential in their program environment.

ESSENTIAL FUNCTIONS:

Case Management:


* The Lead Case Management Supervisor II will provide direction, supervision, and case management for all assigned participants to include: completing intakes, completing an initial plan and assessment, developing and implementing an individual program plan and document progress.

The incumbent will function as part of the interdisciplinary team and develop the individual rehabilitation program.

The Lead Case Management Supervisor II will encourage independence by assisting participants in problem solving by using accepted and innovative rehabilitation techniques and maintaining a proactive environment.

The incumbent will also maintain good working knowledge of Goodwill's policies and procedures as well as state regulations and rules.

Staff Support and Development:


* The Lead Case Management Supervisor II is responsible for all personnel issues for direct reports, including monitoring performance and completing performance evaluations.

As a Team Lead, the incumbent will supervise direct reports and ensure all staff are present for work and held accountable for their job duties.

The Lead Case Management Supervisor II will monitor the work of each staff member for accuracy and appropriateness while ensuring that all case management paperwork for CARF and funding agencies are turned in on appropriate due dates.

The Case Management Supervisor II will also assist with direct reports with problems and conflicts concerning funding agencies, parents, guardians, and consumers.

The incumbent will cover for staff when absent.
