Typical day as a Reablement Worker


* Working closely with the care team to follow support plans, reporting concerns.


* Help customers regain or maintain independence to live well in their own home by carrying out every day daily living activities themselves.


* Carrying our risk assessments, support planning, goal setting and regular reviews.


* Supporting customers who have more complex needs with personal care including bathing, toileting and getting dressed.


* Supporting our customers in their own apartment who have a range of support needs including frailty and dementia.

You bring


* A level 2 or above, or equivalent in social care or being willing to work towards this


* Care Certificate or willing to complete within the first 12 weeks of employment


* If you’ve worked in a CQC care setting previously, that would be great!


* Have a positive, caring can-do attitude, and empathy to really make a difference to our customers lives.


* Previous experience of working with support plans, recording and using information concisely and effectively.


* Knowledge of local community and networks would be brilliant.

Job details


* We provide 24-hour support.


* Night shifts with some lone working, plus regular team meetings and training.
