The Care Manager Supervisor is responsible for the day-to-day supervision of Health Home Care Management or ICM/SCM staff, and is the staff in charge in the Program Director’s absence, both in the office and in the community.

Essential Functions


* Supervises staff in the day-to-day implementation of program services, and provides support and direction to achieve program deliverables and encourage staff performance.

Provides staff with orientation, ongoing trainings, supervision, and disciplinary action when necessary, to ensure high quality performance and retention.



* Ensures that the program delivers its intended results, and also meets all contractual, regulatory, and other specific standards.



* Develops, implements, and communicates expectations to staff to ensure quality services are being delivered.



* Responds to and resolves client and program-related emergency situations.

Oversees the administration of the 24-hour on-call response service, and may share in on-call responsibilities.

Performs the role of Care Manager, when necessary.



* Completes all required documentation, in timelines in accordance with program standards

Qualifications


* Bachelor’s Degree in health or human services field; LMSW, LCSW or LMFT preferred.

Having at least two (2) of the aforementioned years being supervisory experience is preferred.


*  Must have a valid New York State driver’s license and access to reliable transportation.

Pay: $60,405
