Position Summary:
The Integrated Behavioral Health (IBH) clinician provides comprehensive behavioral health services to care for patients with medical and behavioral issues in the primary care environment.

The IBH clinician is integrated directly into the clinical environment and provides direct care and coordination of services in tandem with his/her primary care and/or emergency department colleagues.

Essential Duties and Responsibilities:


* The provision of direct patient care, including psychosocial assessment, brief therapeutic interventions, short term cognitive behavioral therapy, and long term psychotherapy, as appropriate to the clinical environment.


* Clinical care management and oversight for special high-risk populations, as specified by the primary care department within which the BH clinician is integrated.  This includes coordination of services with outside behavioral health providers when necessary and periodic outreach to engage patients in care.


* General practice support, including emergency response to attend to patients in crisis, clinical consultation, behavioral health and SDOH screening, and in-basket management, and support for community education efforts.


* Commitment to reducing social determinants of health for complex needs amongst our patient population.


* Ability to effectively manage clinical practice consisting of a steady patient panel and on-demand integrated behavioral health in primary care.


* Active collaboration with clinical care teams to ensure the effective management of patient needs.


* Connect patients with appropriate resources and services, both within and outside of the primary care setting.


* Monitor patient outcomes by tracking progress using assessment tools and adjusting treatment plans as needed.
