RN MDS Coordinator - Napa Post Acute
Location: Napa, CA
Pay: $110,000-$120,000/year (DOE)
Status: Full-Time

Are you an experienced RN with a strong understanding of MDS and care planning? Napa Post Acute is seeking a detail-oriented RN MDS Coordinator to lead our assessment and care planning processes with accuracy, compassion, and clinical excellence.

What You'll Do:


* Coordinate and oversee the MDS assessment process to ensure timely and accurate submissions


* Develop and update individualized care plans with the interdisciplinary team


* Ensure compliance with all federal and state regulations, including PDPM guidelines


* Collaborate with clinical and administrative staff to optimize resident outcomes and reimbursement


* Provide staff education related to MDS processes and documentation

What We're Looking For:


* Active Registered Nurse (RN) license in California


* Minimum 1 year of MDS experience in a skilled nursing facility


* Strong knowledge of PDPM, CMS regulations, and RAI process


* Excellent organizational, communication, and critical thinking skills


* Passion for delivering quality care to seniors
