? What You'll Be Doing:


* Coordinate and oversee accurate, timely MDS assessments and care plans


* Collaborate with the interdisciplinary team to ensure the highest quality of resident care


* Ensure documentation is thorough, compliant, and reflects the true picture of resident needs


* Help track quality indicators and support QAPI initiatives


* Be the go-to for MDS guidelines, updates, and coordination

? What We're Looking For:


* Current LVN license in good standing (required)


* MDS experience preferred, or strong willingness to learn and grow in this role


* Strong understanding of RAI process and care planning


* Excellent communication, time management, and documentation skills


* A team-first attitude with a genuine passion for elder care


* Availability for a Monday-Friday schedule
