As our Lead Reablement Worker, you'll be at the heart of supporting our customers aged 55+ to stay independent in their own homes.
You'll lead our amazing team of Reablement Workers, helping them deliver person-centred care that makes a real difference.
From supporting people with daily living to coordinating care plans and working with health professionals, you'll be key to helping our customers live well and stay connected to their community.
Typical day as a Lead Reablement Worker
Supporting customers with varying needs, including dementia and frailty, to regain or maintain independence in their daily activities.
Developing and reviewing personalised care plans that focus on reablement, safety and the customer's aspirations.
Providing guidance and leadership to Reablement Workers, ensuring high-quality, person-centred care is delivered.
Managing more complex personal care needs, including stoma and catheter care, while promoting customer choice and dignity.
Working closely with external professionals, like health and social care teams, to make sure customers get the best support possible.
Using creative solutions, including assistive technology, to empower customers to live independently for as long as possible.

You bring
Experience in a similar role, with a strong understanding of reablement and empowering customers to live independently.
Adult Care Level 3 diploma (or level 2 with relevant experience and willingness to study for level 3)
Great leadership skills, with experience in mentoring or guiding teams in a care setting.
A solid understanding of CQC standards, risk management and safeguarding, ensuring customer wellbeing and safety.
The ability to build strong, trusting relationships with customers, families and professionals to deliver holistic, person-centred care.

Job Details
Working on a rota basis, with shifts ranging between 7am to 10pm, taking your turn to work days, evenings, weekends and bank holidays.
You may from time to time be required to do some occasional nights during the initial set up of the services or in emergencies.
We need you to be flexible to meet the needs of our customers, but in return we’ll give you the time you need to spend time doing the things that matter to you!
