? What You'll Be Doing:


* Coordinate and complete accurate Minimum Data Set (MDS) assessments


* Focus on long-term residents—get to know the people behind the paperwork


* Collaborate with interdisciplinary team members to ensure compliance and excellent resident care


* Use your keen eye for detail to capture clinical information and support quality outcomes


* Maintain documentation that aligns with federal and state regulations

✅ What We're Looking For:


* At least 1 year of experience as an LVN (required)


* Prior MDS experience a big plus (but we're open to training the right person!)


* Strong assessment, documentation, and organizational skills


* A compassionate, team-oriented mindset


* Preferred schedule: Wednesday-Friday, but we're flexible for the right fit
