Patient Care Coordinator

Hourly Wage starts at $19

Key Responsibilities:


* Scheduling & Follow-up:
+ Schedule post-operative and follow-up appointments for patients in a timely and efficient manner.
+ Coordinate appointment reminders and follow-up communications.
+ Ensure all pre- and post-operative instructions are clearly communicated and documented.


* Patient Referrals:
+ Process inbound and outbound patient referrals in accordance with clinical protocols.
+ Track referral status and ensure timely completion of referral appointments.
+ Maintain accurate records of all referral communications.


* Medical Record Audits:
+ Conduct regular audits of patient medical records to ensure accuracy, completeness, and compliance with HIPAA and organizational standards.
+ Identify and report discrepancies or documentation gaps to the Medical Records Director.


* Communication & Coordination:
+ Serve as a point of contact for patients regarding care coordination, appointment status, and required documentation.
+ Collaborate with providers, clinical staff, and administrative personnel to ensure continuity of care.
+ Address patient questions and concerns with professionalism and empathy.

Qualifications:


* High school diploma or equivalent required


* Minimum of 2 years experience in a healthcare setting, preferably in a coordination or administrative role.


* Proficient in medical terminology and understanding of clinical workflows.


* Experience with electronic medical record (EMR) systems required.


* Exceptional verbal and written communication skills.


* Strong attention to detail and accuracy in data entry and recordkeeping.


* Ability to manage multiple tasks and priorities in a fast-paced environment.


* Knowledge of HIPAA compliance and confidentiality requirements.

Supervision:

This position reports directly to the Medical Records Director and works closely with both the administrative and clinical teams
