Program Specialist - Lifesharing (Erie)

Pay: Hourly-Exempt Status - $21.32

Hours: 40 hours/week - must be flexible; some On-Call

Overview:

Are you passionate about making a difference in the lives of individuals with intellectual disabilities or autism? We are seeking a Residential Family Living Program Specialist to join our team in delivering compassionate, person-centered care through our Lifesharing model.

What you’ll bring:

A typical day-to-day may include:


* Coordinating and conducting monthly visits to Lifesharing homes to support individuals and providers


* Developing and updating Individual Support Plans (ISPs) in collaboration with stakeholders


* Delivering training to Lifesharing providers on health, safety, and program expectations


* Monitoring service delivery and ensuring compliance with state and county regulations


* Supporting individuals with budgeting, benefits management, and financial education


* Participating in intake, admission, discharge, and annual meetings to ensure continuity of care


* Conducting assessments and health screenings, and tracking fire drills and self-preservation activities


* Maintaining accurate documentation in CareLogic and overseeing individual spending and reporting

What you’ll have:


* A master’s degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism.


* A bachelor’s degree from an accredited college or university and 2 years’ experience working directly with individuals with an intellectual disability or autism.


* An associate’s degree or 60 credit hours from an accredited college or university and 4 years of experience working directly with individuals with an intellectual disability or autism


* Valid driver’s license


* Strong understanding of regulatory standards and documentation practices


* Ability to work effectively with diverse stakeholders and manage multiple responsibilities
