? Key Responsibilities:


* Develop and implement individualized care and discharge plans in collaboration with the interdisciplinary team


* Serve as a liaison between residents, families, and community agencies


* Conduct psychosocial assessments and maintain thorough documentation


* Participate in care conferences and ensure residents' rights and needs are respected


* Provide support during admissions, discharges, and transitions of care


* Promote resident well-being through advocacy and person-centered support

✅ Qualifications:


* Bachelor's degree in Social Work or related field (required)


* Minimum 1 year of experience in a skilled nursing or long-term care setting (preferred)


* Knowledge of discharge planning, Medicare/Medicaid regulations, and community resources


* Strong organizational, communication, and documentation skills


* Passion for working with elderly and post-acute populations
