Ridgeway Post Acute is looking for a compassionate and organized Facility Liaison to support and help lead our Social Services Department.

What You'll Do:


* Support the Social Services Director in coordinating care plans and discharge planning


* Serve as a key point of contact for residents and their families


* Help ensure smooth transitions throughout the care continuum


* Advocate for resident needs and assist with emotional and psychosocial support


* Collaborate with nursing, therapy, and administrative teams to promote positive outcomes

What We're Looking For:


* Experience in a skilled nursing or healthcare setting preferred


* Strong communication, interpersonal, and problem-solving skills


* Compassionate, empathetic, and able to navigate sensitive conversations


* Organized and able to manage multiple priorities with care and professionalism
