What You'll Do:


* Coordinate and oversee all social services functions in the facility


* Support residents and families through the admissions, care planning, and discharge process


* Conduct psychosocial assessments and develop individualized care plans


* Build strong relationships with residents, families, and community resources


* Help ensure compliance with state and federal regulations

What We're Looking For:


* Experience in a skilled nursing facility or healthcare setting preferred


* Strong organizational, communication, and problem-solving skills


* Knowledge of discharge planning, care transitions, and social services documentation


* Ability to multitask and maintain a calm, compassionate presence under pressure
