Lincoln Meadows Care Center is seeking compassionate Physical Therapists (PT) to join our dedicated team in providing high-quality care to our residents.

We have part-time positions available with weekend availability required.

Key Responsibilities:


* Provide physical or occupational therapy services to elderly residents


* Develop and implement personalized therapy plans to enhance resident mobility, independence, and quality of life


* Collaborate with a multidisciplinary team to ensure comprehensive care


* Maintain accurate documentation of therapy sessions and progress


* Support residents and their families through education and compassionate care

Qualifications:


* Valid Physical Therapist (PT) license in the state


* Experience in a skilled nursing facility or with elderly patients is a plus


* Strong interpersonal and communication skills


* Ability to work independently and as part of a collaborative team
