* Evaluate and develop individualized treatment plans for residents


* Provide direct therapy services and monitor progress


* Collaborate with nursing, OT, and SLP for interdisciplinary care


* Document treatments and outcomes per facility and regulatory standards

✅ Requirements:


* Active California Physical Therapist (PT) license


* Strong communication, organization, and teamwork skills
