? Pay: $80,000-$100,000/year, DOE

Midtown Oaks Post Acute is seeking a detail-oriented and experienced MDS Coordinator to lead our facility's assessment and care planning processes.

What You'll Do:


* Oversee the completion and accuracy of MDS assessments and care plans


* Ensure compliance with federal and state regulations


* Collaborate with interdisciplinary teams to support optimal care outcomes


* Track and manage Medicare and Medicaid reimbursement processes


* Educate staff on documentation and MDS processes

What We're Looking For:


* Active RN or LVN license in California


* Prior MDS experience required (minimum 1 year preferred)


* Knowledge of PDPM, RAI process, and regulatory guidelines


* Strong communication, organizational, and analytical skills
