What You'll Do


* Deliver person-centered nursing care for residents living with Alzheimer's and related dementias


* Complete focused assessments, medication administration, and treatment orders per care plan


* Lead CNAs during the AM shift, modeling best practices in dementia communication and redirection


* Collaborate with the RN Supervisor, interdisciplinary team, and families on individualized goals


* Document accurately in the EMR and report changes in condition promptly


* Champion infection prevention, fall reduction, and restraint-free practices

What We're Looking For


* Current, unencumbered California LVN license


* Experience in skilled nursing or memory-care preferred (new grads with strong passion welcome)


* Ability to adapt care strategies for behaviors related to dementia


* Clear, compassionate communication and strong teamwork mindset


* CPR/BLS certification (or willingness to obtain on hire)
