Position: MDS Licensed Vocational Nurse (LVN) - Full-Time
Annual Salary: $71,000 per year
Schedule: Full-Time | Day Shift

About the Role:
Are you a detail-oriented LVN with a passion for documentation and resident care planning? Sundance Creek Post Acute is looking for a dedicated MDS LVN to join our team and help ensure accurate assessments and compliance with regulatory standards.

You'll play a key role in coordinating care plans and supporting our interdisciplinary team.

What You'll Do:
? Complete and manage MDS assessments and care plans
? Ensure accuracy and timeliness of documentation for reimbursement and compliance
? Collaborate with nursing, therapy, and administrative teams
? Participate in care plan meetings and audits
? Monitor changes in resident conditions and update assessments accordingly

What We're Looking For:
✔️ Active LVN license in good standing
✔️ Experience with MDS 3.0 and care planning preferred
✔️ Strong attention to detail and organizational skills
✔️ Excellent communication and teamwork abilities
✔️ Familiarity with PDPM and Medicare/Medicaid regulations is a plus
