Location: Whitney Oaks Care Center - Carmichael, CA
Pay: $42-$48/hr, DOE

Key Responsibilities:


* Coordinate and manage the MDS/RAI process


* Ensure accurate and timely completion of assessments


* Collaborate with interdisciplinary team for care planning


* Monitor quality indicators and assist with regulatory compliance


* Serve as a resource for clinical documentation improvement

What We're Looking For:


* Active California LVN or RN license


* Prior experience in MDS coordination and PPS preferred


* Knowledge of PDPM and care planning


* Strong communication, assessment, and analytical skills


* Team player with a positive, solution-focused attitude
