Position Summary:
The Family Partner position is grant-funded in partnership with the Boston Public Health Commission and is part of an initiative to create a network of Community Healing Response Networks (CHRN) within the City of Boston.

The CHRN provides comprehensive trauma response services and community healing programming to residents of the City who are impacted by community violence.

The Family Partner works in close collaboration with our Director of Community Healing Response Services, Care Navigation team and community partners within the East Boston CHRN to provide recovery services, ongoing healing modalities and support to individuals and families.
Under the direction of the Director of Community Healing Response Services, the Family Partner conducts resource assessments and collaborates with behavioral health and/or medical staff to obtain community resources for patients and families who have experienced trauma.

Within developed area of expertise, provides orientation and ongoing training related to community resources to staff, medical providers, patients and families.
The Family Partner responsibilities include: outreach and engagement to those impacted by community violence and other forms of community-based trauma, case management, assistance with supports and service linkage; participation in briefing calls; conducting home visits as needed; ensuring active, accurate and consistent communication with other staff and partners; and participation in training and supervision.

The Family Partner may function as a compassionate presence at community meetings, funerals, or events and conduct outreach to families and other agencies in the community affected by traumatic events.
