Napa Valley Care Center is seeking a detail-oriented and organized MDS Assistant to support our clinical team in managing resident assessments and care documentation.

If you have a strong understanding of skilled nursing documentation and a passion for accuracy and compliance, this could be the perfect fit!

Position Highlights:


* Assist the MDS Coordinator with timely and accurate completion of MDS assessments


* Ensure proper documentation to support coding and reimbursement


* Help monitor care plans and interdisciplinary team input


* Stay current on Medicare/Medicaid guidelines and PDPM standards

Qualifications:


* LVN license in CA


* Experience with MDS 3.0 in a skilled nursing setting strongly preferred


* Strong attention to detail and organization


* Familiarity with PDPM and CMS compliance requirements


* Team-focused mindset with excellent communication skills
