What You'll Do:


* Deliver personalized therapy treatments to short-term rehab and long-term care residents


* Collaborate with our interdisciplinary team to optimize patient outcomes


* Document progress and adjust care plans as needed


* Educate residents and families on continued care strategies

What We're Looking For:


* Valid California Physical Therapist license


* Experience in skilled nursing or post-acute care preferred


* Strong communication and documentation skills


* Team player with a resident-first mindset
