Social Services Assistant

Cooper River Post Acute is a magnificent 176 bed skilled nursing facility located in the heart of Mount Pleasant, SC.

We're seeking an experienced Social Services Assistant who will play a vital role in supporting the emotional and psychosocial well-being of our residents.

Key Responsibilities

As a Social Services Assistant, you will:


* ? Assist in the development and implementation of resident-centered social services programs


* ? Support residents and families throughout the admission, care, and discharge process


* ? Maintain timely and accurate documentation in compliance with South Carolina DHEC and federal regulations (including CMS)


* ? Participate in interdisciplinary care planning to address resident needs and preferences


* ?️ Respond to resident concerns and promote their rights and dignity


* ? Assist with MDS 3.0 assessments and care plans as appropriate

Qualifications

✅ Required:


* Minimum of 1-2 years' experience in a nursing home or long-term care facility


* Working knowledge of State of South Carolina DHEC regulations and CMS standards


* Excellent interpersonal, organizational, and documentation skills

✅ Preferred:


* Experience with MDS documentation and care planning software


* Bilingual (Spanish or other language helpful)


* Associate's or Bachelor's degree in Social Work, Psychology, Human Services, or a related field (preferred)

Assist the Social Service Director/Consultant in the planning, developing, organizing, implementing, evaluating, and directing of the social service programs of this facility.

Meet with administration, medical and nursing staff, and other related departments in planning social services, as directed Assist in the development, administering, and coordinating of department policies and procedures.

Review department policies and procedures, at least annually, and participate in making recommended changes.

Assist in developing and implementing policies and procedures for identifying the medically related social and emotional needs of the resident.

Participate in community planning related to the interests of the facility and the services and needs of the resident and family.

Assist residents in achieving the highest practicable level of self care, independence and well being.

Assist with providing medically related social services so that the highest practicable physical, mental and psychosocial well being of each resident is attained or maintained.

Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director.
