LPN - Weekends (day/NOC)
Job Summary:
The Licensed Practical Nurse is responsible for providing direct patient care and supporting the nursing team in a Skilled Nursing Facility.
The LPN will deliver compassionate care to residents, assist with daily living activities, administer medications, monitor vital signs, and collaborate with the healthcare team to ensure high-quality care.
This role is essential in maintaining a safe, comfortable, and caring environment for residents, while ensuring compliance with regulatory requirements.
Key Responsibilities:
* Resident Care
* Provide direct, hands-on nursing care to residents, including administering medications, dressing changes, and treatments as prescribed by physicians.
* Monitor and document residents' vital signs (blood pressure, pulse, temperature, etc.), noting any changes in condition and reporting to the Charge Nurse or Director of Nursing (DON).
* Assist residents with daily activities such as bathing, dressing, grooming, and eating, as needed.
* Observe and report any physical, mental, or emotional changes in residents' conditions to the nursing supervisor.
* Help implement individualized care plans in collaboration with the nursing and medical teams.
* Assist with admission, transfer, and discharge procedures for residents, ensuring accurate documentation.
* Medication Administration & Documentation
* Administer medications as prescribed by physicians and in accordance with facility protocols.
* Maintain accurate and timely documentation of medication administration and any changes in the residents' conditions.
* Ensure proper storage and handling of medications, including controlled substances, following all regulatory guidelines.
* Collaboration & Communication
* Communicate effectively with residents, families, and other healthcare providers to ensure coordinated care.
* Act as a liaison between the nursing team and other departments, ensuring that all aspects of care are being met.
* Report changes in residents' health conditions to the Charge Nurse and assist in providing follow-up care as needed.
* Compliance & Quality Care
* Follow all facility policies, state regulations, and federal guidelines to ensure compliance with healthcare standards.
* Participate in care planning meetings and assist in the development of care plans to improve residents' outcomes.
* Participate in facility audits, quality improvement initiatives, and resident satisfaction surveys.
* Training & Professional Development
* Maintain current licensure and stay up to date with best practices in nursing care through continuing education and training opportunities.
* Assist in the training of nursing assistants (CNAs) and new nursing staff as needed.
* Participate in team meetings, in-service training, and staff development opportunities.
