Clinical Coordinator RN

Position: Clinical Coordinator RN

Pay: $70000 - 77000 yearly Depending on Experience

The Clinical Coordinator assist DON/Patient Care Manager (PCM) with coordinating all aspects of patient/family care and support and facilitates communication and interaction among personnel providing services to terminally ill patients.

Responsibilities:

Assist PCM with coordination of the patient care aspects of the Hospice Program and the Interdisciplinary Team (IDT).
Ensures referral/admission process is completed within Caris HealthCare's customer service standards.
Assist PCM with assignment of patient care staff based on competency of available personnel.

Monitors scheduling of patient care staff to provide adequate coverage to include 24 hour call and assures adequate staffing resources are available to meet patient care requirements.
Assist PCM to assure that nursing services delivered by hospice employees and by individuals employed through contractual agreements are provided in accordance with established policies of the hospice.
Assist PCM with supervision of periodic reviews of patient medical records, employee visits, and daily activity reports to assure conformance with agency standards, licensure requirements, and professional community health nursing standards.

Checks documentation for accuracy, and consistency with payor requirements and company policy.
Assist PCM with planning of team conferences and IDT meetings.
Assist PCM with arrangement of hospice and supportive services for patients/caregivers and coordinates such services.

Ensures that medical supplies, durable medical equipment, and medications are available and secured for patients when needed.
Assist PCM and ADM with recruiting, hires, and directing the training of field staff in coordination with the Support Center.

Demonstrates effective counseling skills in employee relations.
Assist PCM to assure the delivery of needed and appropriate services in accordance with agency and government policies through effective supervision of hospice staff.
Ensures deviations from policy are reported through the occurrence reporting process, with necessary follow-up steps taken to ensure compliance.Responsible for compliance with the Caris safety and disaster plans in the event of emergencies, disaster, etc.
Ensures employees are trained on occupational exposure upon hiring and prior to initial assignment to tasks where occupational exposure may occur and ensures training is conducted on the exposure control plan annually or as needed.Assist PCM to handle staff, physicians, patient, inter-departmental complaints, and problems, providing accurate and timely follow-up, and documents problems and complaints per company policy.Assist PCM to complete systematic collection, review, and evaluation of data pertaining to hospice aspects of care.
Assist PCM with completion of all patient medical record reviews.Exhibits knowledge of labor relations, laws and regulatory issues.
